The prostate gland is the size of a walnut and is located between the penis and the bladder and just in front of the rectum. The urethra runs through the center of the prostate, from the bladder to the penis, and then allows urine to flow out of the body.

The prostate gland helps produce seminal fluid and is part of the biologically male reproductive system. Its incidence is age-related: it is estimated that nearly 50% of men at the age of 60 and almost 90% by the ninth decade develop histological BPH. It starts off the size of a walnut, growing gradually throughout a man’s life to the size of a golf ball, and then the size of a lemon or apricot.

  • Dribbling of urine.
What does the prostate do?

A functioning prostate is not required to live, but it is essential for fertility.

1. It helps in semen production:- The primary function is the provision of prostatic fluid to the semen. At least 20–30 percent of the semen volume is contributed by the prostate. In addition, 5 percent of the semen volume comes from the testicles, while 50–65 percent comes from the seminal vesicles.

2. The prostate closes the urethra during ejaculation:-When ejaculating, the prostate contracts and releases prostatic fluid into the urethra. The prostatic fluid mixes with sperm fluid and cells. The fluid comes from the seminal vesicles into the urethra, where it mixes with sperm cells to form semen. The body then expels this semen.

3. Hormone metabolism:- The prostate needs androgens. Androgens are male sex hormones, like testosterone. Without this, the prostate cannot function correctly. There’s an enzyme in the prostate called 5-alpha reductase. This enzyme converts testosterone into dihydrotestosterone, the biologically active form of testosterone.

Dihydrotestosterone is involved in the normal development and functioning of the prostate. In addition, dihydrotestosterone is essential for developing facial hair and other secondary sexual characteristics in the developing male.

What are the reasons for prostate enlargement?

The cause of enlargement of the prostate is very controversial in research, but I would like to highlight a few things commonly observed in a long study.

  1. Hyperinsulinemia has been suggested to be causally related to the development of BPH.
  2. Insulin Resistance:-Insulin-resistance syndrome includes a group of disorders, such as obesity, dyslipidemia, sympathetic overactivity, and hyperinsulinemia, and each is individually reported as a risk factor for the development of BPH 
  3. Insulin-like Growth Factor:- this is known to act as a growth factor 
  4. Increase in obesity-related estrogen levels:-  hormonal imbalance between testosterone and estrogen 
How is prostate Enlargement diagnosed?

Early diagnosis of BPH is important because if left untreated it can lead to urinary tract infections, bladder or kidney damage, bladder stones, and incontinence. Distinguishing BPH from more serious diseases like prostate cancer is important.

  • Trans abdomen sonography
  • Uroflowmeter
  • PSA
  • Rectal examination or Cystoscopy

  • urinating as soon as you feel the urge
  • going to the bathroom to urinate, even when you don’t feel the urge

  • reducing your stress level, as nervousness can increase the frequency of urination
  • exercising regularly, as a lack of exercise can aggravate your symptoms
  • learning and practicing Kegel exercises to strengthen your pelvic muscles

  • hormone reduction medications:-  reduce the levels of hormones produced by the prostate gland

  • Transurethral resection of the prostate (TURP)
